New Patients

Register as New Patient

On registration, every patient is given a named GP who is responsible for their care and support, this does not mean you have to see them unless you choose to. We aim to provide continuity of care and where possible we will book you with the same GP for any ongoing care, especially if you have a long-term medical condition. If your have an urgent problem that requires an on the day appointment or wish to see a GP sooner, you may be offered an appointment with another GP.

If you are a new patient on long-term medication, please make an appointment to see a doctor within two weeks of joining the practice, so that your needs are clearly documented before you actually need a prescription.  It may take a while for your old records to arrive.

When patients request to join our practice list, we do not discriminate on the grounds of race, gender, social class, age, religion, sexual orientation, appearance, disability or medical condition

It is normal practice that patients moving out of the practice’s catchment area are asked to register with another GP practice nearer their new address, we do this as we may not be able to provide the care required for patients with complex health issues or if you require support form services outside of our catchment area.

If you would like to register with Bridgewater Surgeries, please complete the form below, you will be sent a health questionnaire once we have received and processed your registration details.
